MIH is teaming with the Maryland Department of Disabilities (MDOD) to help revise the State Disabilities Plan. In this survey, you will have the opportunity to tell us what programs and issues you would like to see addressed in the State Disabilities Plan. Your feedback will help guide MDOD’s focus as state agencies work to deliver high-quality disability programs and services to Marylanders with disabilities.
The State Disabilities Plan is a guide that helps all levels of government and all parts of the community coordinate efforts to improve the lives of Marylanders with disabilities, their families, and caregivers. The plan is created in collaboration with state agencies, community members, and other stakeholders.
This survey is completely anonymous. No personal data will be collected as part of this survey. You can answer as many or as few questions as you would like. If you have any questions about this survey or the State Disabilities Plan, please contact the Maryland Department of Disabilities at 410-767-3660 or firstname.lastname@example.org.
