Sarah Hochbaum, Community Living Coordinator

Sarah Hochbaum became the Community Living Coordinator at Maryland Inclusive Housing in May of 2022.

Hochbaum grew up in Columbia, Md. And has a Bachelor’s and Master’s Degree in Social Work from Adelphi University. Her passion for housing began when she started working at Green Door in Washington D.C., which supported the homeless population living with mental health and substance abuse disorders.

Sarah continued her work at Bronxworks in Bronx N.Y., as a social worker in their transitional housing program. She was previously employed by The Coordinating Center, contracted through Montgomery County’s Department of Health and Human Services Housing Initiative Program, as well as a Supports Planner for the Home and Community Based Programs contract.

Hochbaum provided case management services to individuals with chronic medical conditions who were experiencing homelessness. She was tasked with assisting individuals with obtaining and maintaining housing, as well as helping them navigate rental subsidies, social service programs and community resources to address their needs. Most recently, she served as a case manager at St. Vincent De Paul of Baltimore, working towards the goal of housing individuals who had experienced homelessness due to the COVID pandemic. Sarah collaborated bi-weekly with the Baltimore City Mayors Office of Homeless Services to reach this goal.
